Where to get Bottle Soul Fragments and a regular Armor Appearance Stone?

How to obtain these items? They don't have a page on wiki...

I know there's some 'upgraded' version in the L2 Store but it's limited, I was thinking if there's a permanent one. Also is it possible to bestow e.g. a C-grade appearance on the R95 items? E.g. to make Seraph armor look like a Karmian?

 

If I am not mistaken regular armor appearence scrolls are available in grocery store. These are used to change appearance to armor of lower grades - yes you can have seraph looking like karmian but you have to sacrifice karmian items (if you want appearance of a S80 for example, prepare some arena). If not they drop from some instances and are minor rewards in numerous events.
For the bottle fragments, if they don't drop from the bosses themselves they are only in caskets which are not yet introduced in Ramona

 

There are some inaccuracies in your post.

Currently there are no such a fragments for Orfen, Freya, and Baium. There are fragments only for AQ, Frintezza, Antharas and Valakas, and as mentioned those are comming from L2Store's Shabby basket. You can get the full bottles from the bosses, and if you are lucky, from the treasure chests sold by the Aden reconstruction NPC in Giran.

As you can see Armor and Weapon modify scrolls are dropping from some bosses. In the grocery store there are only appearance removal scrolls. You can only put the same kind of armor, eg. robe can only have robe appearances, and it cannot be higher grade than the actual item. And you need to change the appearance for all the visible armor parts (except for the special ones, like the ones in L2Store), to have the all Karmian look, otherwise you will end up with a mixed appearance. So you need 4 scrolls (tunic, stockings, boots, gloves) to get the full Karmian set appearance. If the source is a one piece armor (eg. Avadon robe), you only need 3 scrolls. And, as mentioned, the source item will be consumed by the scroll.
